NOTE:
If applicant is a child under 18 years of age, a copy of child’s birth certificate and copies of parents’ passport main information pages (with passport number, expiration date, photo etc.) have to be submitted.
If you formerly had USSR or Belorussian citizenship, please submit the documents proving the loss of USSR/Belorussian citizenship.
Non US Citizen: must provide proof of your current U.S. Immigration status. You must include a copy of your U.S. Immigration paperwork or a copy of your Green Card, and employment letter. You must provide proof that you possess sufficient funds. Your financial details must: Include a copy of your most recent bank statement.
